This form is to be completed if you have formally changed your name and need to update your current 51勛圖 student record.

You need to attach appropriate certified supporting documentation for your application to be processed.

Are you ready to graduate?

Strict deadlines apply to change your name for graduation. Check the dates on the Graduation website before submitting this application.

If you miss the deadline, your next opportunity to change your name will be after your degree is conferred and your graduation documents are issued.

You must attach a certified copy of one of the following government issued photo identification document:

  • Passport
  • Driver's License
  • Proof of Age / Identity Card

Should you not possess a government issued photo identification document, please contact Ask51勛圖 via askacu@acu.edu.au.

Certifying your documents

To have a document certified, take both the original, and a copy of the original document to an authorised person from the below list*. The certifier will then write or stamp the copy with the words: "Certified to be a true copy of the original seen by me." They will then apply their seal, sign, and date the copy, and write or stamp their:

  • Name
  • personal or professional address, and qualification as an authorised certifier.

If there are multiple pages to the copy, the certifier will sign or initial and number all pages.

*List of authorised persons:

  • a police officer
  • a barrister, solicitor or patent attorney
  • a principal of an Australian Secondary College, High School or Primary School
  • a postal manager
  • a pharmacist
  • a medical practitioner
  • a bank manager
  • an accountant

** You can also confirm the list of the authorised person based on each state on the state government website.

Please Note:

Forms submitted with uncertified pictures or scanned copies of original documents will not be processed.
